Refer your patient to Gabbi
- During a visit, explain the importance of breast cancer risk assessment to your patient
- Refer the patient to Gabbi via e-fax at (833) 973-4606 or via your EMR

Patient has a virtual visit with Gabbi
- Gabbi reaches out within 24 hours to schedule your patient for a virtual visit
- During a 30-minute virtual visit, the Gabbi breast specialist assesses your patient's risk, designs a care plan, and orders any necessary testing or imaging

Patient gets any needed testing or imaging
- If tests or imaging are needed, Gabbi will schedule your patient within 14 days at convenient, in-network locations
- Gabbi will follow up with your patient to ensure they complete any needed testing or imaging

Gabbi supports ongoing care, keeping you in the loop
- Your patient returns to Gabbi to review and discuss any test results
- As the referring provider, your are notified if any positive or abnormal results are received
- Gabbi continues to manage their breast health as needed
